Description

Case Planner will provide case management services to adolescents and/or young adults connected to the foster care system.

Individual will be responsible for helping adolescents develop independent living skills toward becoming self-sufficient to live on their own. Experience with foster care system, ability to effectively utilize community resources, strong engagement skills and excellent documentation skills are preferred.

The case planner will have the ability to work with diverse populations from various economic and social backgrounds and be culturally sensitive in nature. 

Applicants must possess a BSW/BA/BS in social work or related field.  EOE
